No Show and Cancellation Policy

Your appointment time is reserved just for you. A late cancellation or missed visit leaves a hole on the therapists’ day that could have been filled by another patient. As such, we require 24 hours notice for any cancellation or changes to your appointment. Patient who provides less than 24 hours notice, or miss their appointment, will be charged a cancellation fee to the card on file. If an emergency situation arises, please let us know to allow us to make adjustments and corrections accordingly. We appreciate your respect of the therapist’s time.
Cancellations are allowed up to minimum 24 hours before your appointment. All late cancellations and no-show appointments are charged a $20.00 fee.
Late Arrivals: Appointment times have been arranged especially for you. Unfortunately we cannot provide late arrivals with extension of scheduled appointment.
Same Day Appointment: are considered fixed and cannot be changed or cancelled once booked.

Extended Health Benefits: Massage will be covered by insurance, let us submit your insurance claim on your behalf. Please contact us with any other questions or concerns you may have.

PLEASE NOTE:
o Insurance benefits Do Not cover missed appointment fees.
o Desjardin Insurance only allows payment to be sent to the plan member. Clients will need to pay at the time of the appointment, but we submit the claim on your behalf for reimbursement to you.
o Committed Hands Massage Therapy (CHMT) cannot directly bill for two (2) hour massages to Sunlife Insurance due to restrictions within their system.
o CHMT is not able to issue Insurance receipts when redeeming a gift card for treatment. It is against policy to issue a false receipt or charge documentation or name on the receipt. CHMT does not control the policies made by CMMOTA and the insurance companies. We apologize for any inconvenience, but we are complying with the regulations as they have been stated to us.

Respecting your privacy is a priority for the Committed Hands Massage therapy. We keep in confidence personal information about you and the services you have with us to provide you to meet your health and wellness objectives. To meet these objectives, we collect, use and disclose your personal information for purposes that include: claims adjudication; protection against fraud, errors or misrepresentations; meeting legal and regulatory requirements. The only people who have access to your personal information are my employees, and third-party service providers. We will also provide access to anyone else you authorize. You can ask for the information in our files about you and, if necessary, ask us in writing to correct it.

Do you sell personal information to third parties?
No, Committed Hands Massage Therapy (CHMT) does not sell personal information or customers lists to other companies regardless of their intentions.
When is my information disposed of?
CHMT disposes of personal information in a secure manner when it is no longer required for the purposes identified in your consent.
